This species is very similar to Ladenbergia stenocarpa and L. klugii. These were distinguished by Andersson (1997) by details of pubescence, and small differences in the size of the flowers and fruits, and degree of lobing of the calyx limb, for ca. half its length vs. for more than half its length. Andersson knew all of these from relatively few specimens, and with more material now available the size range of the flowers and fruits is now broader, the pubescence seems to be more variable, and the degree of lobing of the calyx limb seems to be a little more variable.
However there are some well marked forms or perhaps distinct taxa within this group of plants; field work is probably needed to understand species limits here. Chilquillo et al. (2019) circumscribed Ladenbergia acutiflora more narrowly than done by Andersson, and included many of his specimens instead in Ladenbergia graciliflora and separated some other, shrubby plants as Ladenbergia siranensis.
