This species is characterized by rather small leaves that are generally oblanceolate, a brown drying color, and cymose inflorescences borne at nodes along the stem and also at the stem apex; it is the most commonly collected species of Psychotria with axillary inflorescences in Madagascar (Bremekamp's group I). It is similar to Psychotria oreotrepehs and Psychotria glaucifolia, and these are apparently not separated from these by reproductive characters and perhaps not distinct. These are arbitrarily separated here based on leaf form and inflorescence position, in particular Psychotria paradoxalis here includes the plants with leaves widest above the middle and a terminal inflorescence present. This species was separated by Bremekamp in part by this presence of a terminal cyme along with axillary inflorescences, but the type collection of one of the species he separated by its wholly axillary inflorescences, Psychotria glaucifolia, also has terminal inflorescences developed on some stems.
